Lab requisitions/kit draw forms must be filled out completely and emailed or faxed to us along with a copy of the insurance card (front/back). This helps us ensure proper equipment and preparation for your specimen collection. Follow your provider’s instructions (fasting or drinking plenty of water.)
After staff has arrived to perform the blood draw your specimen will be delivered to the designated laboratory for specimen processing.
Specimens are processed under the protocol of the laboratory’s policy. Results are sent directly to the ordering provider.
No orders are scheduled or drawn without a physician’s order.
